The professional system of electromyography/nerve localization at the site of intramuscular injection MIST is a portable wearable device for injection control (analysis of the accuracy of insertion of the injection needle during injection manipulations), conducting BFB training sessions (BFB sessions via the EMG channel for paralyzed and weakened muscles), physiotherapeutic neuromyostimulation and electrotherapy (myostimulation sessions for muscle relaxation or activation).

The professional version of the MIST device enables to work in 4 possible modes:
First of all, this system configuration is intended for specialists in botulinum therapy. The device allows for myographic control by introducing injection needles for localization of muscle motor points and topical localization of target muscles. This is necessary when injecting muscle relaxant drugs: botulinum toxins Dysport, Botox, Xeomin, Lantox, etc., local analgesic anesthetics and anti-inflammatory drugs.
In the «Amplifier» mode the device is used to analyze the accuracy of insertion of injection needles during injection procedures.
The main purpose of the mode is localization of muscle motor points and topical localization of target muscles for injections of muscle relaxant drugs: botulinum toxins (Dysport, Botox), local analgesic anesthetics (when performing muscular blocks) and anti-inflammatory drugs. Precise drug injection into the motor point area helps to increase the therapeutic effect with significantly lower doses of the administered drug. The device can also be used for rapid quantitative analysis of muscle contraction (using skin electrodes) during kinesiological diagnostics when studying movement patterns.
The device can be equipped with special needles for administering a drug at the time of muscle activity recording. The needles used are disposable monopolar needle electrodes intended for targeted administration of botulinum toxins. They allow for the administration of botulinum toxin under the control of a continuously recorded EMG signal.
Bioelectrical activity of muscles (EMG activity, myogram) is output to speakers (headphones or portable speakers). Based on the specific audio characteristics of the myogram, a physician can verify the injection site (i. e. the location of the needle tip directly at the motor point – in the area of the highest density of end plates).
EMG signal stability during injections is ensured by the high quality of electrodes and the technologies of internal soldering of the cable and the needle.
Needle electrodes for Botox injections under EMG control are available in different lengths: 37 mm, 50 mm, 75 mm.
Disposable needle electrodes and hypodermic needles are compatible both with Luer-Lock and Luer-Slip syringes.

In the «Stimulator» mode the device allows (in addition to recording the myogram) to perform electrical stimulation of the muscle under examination. Electrical discharges generated by the device cause an action potential, a visible muscle contraction and a corresponding motor effect (especially in the relatively small muscles of the face, forearm, hand, etc.).
A current discharge is supplied to the needle tip, causing an evoked muscle response. The subsequent injection is performed into the muscle after preliminary determination of its function, which significantly increases the accuracy of administration and the therapeutic effectiveness of the procedure. The device is used in the diagnostic and therapeutic work of the neurological and diagnostic department.
In the «BFB Trainer» mode the device is used to conduct individual therapeutic sessions of EMG-BFB training of paralyzed and weakened muscles (consequences of strokes, cerebral palsy, injuries to nerve trunks and spinal cord, facial nerve neuritis, postural disorders, etc.).
The patient can conduct training sessions independently without the participation of a physician, preferably after special training by an instructor. The device is both a load dispenser and a "mirror" displaying the actual muscle contractions of the muscles being trained. The fundamental feature is the ability to register minimal muscle contractions that are invisible to the eye (in severe paralysis).
The device does not have any active electrical effect on the body. On the contrary, it serves as a sensor-antenna that records the patient’s own bioelectrical muscular activity – a myogram, which is the equivalent of the power of muscle contractions. By controlling it, the patient has the opportunity to train the affected muscles in conditions where the physiological connection between the muscle and the brain is disrupted. By controlling the power of muscle contractions, just as when performing therapeutic physical exercises, the patient trains the strength of the muscle and creates a "new" reflex connection between the paralyzed muscle and the affected structures of the nervous system. This principle of obtaining additional information about qualitative and quantitative characteristics of the functional activity of the organ being trained in real time is called biofeedback.
When addressing tasks of posture correction and during kinesiotherapeutic interventions, this method is used to form an optimal motor stereotype.
In the «Electrotherapy» mode the device is used to conduct sessions of special myostimulation for the purpose of relaxation or activation of muscles. The mode includes 7 special electrical stimulation programs developed by qualified specialists.
