Callibri BeFit PRO is an innovative model of hardware and software systems for rehabilitation of pelvic floor muscles using the biofeedback (BFB) method. The system is designed for the use by physicians from various fields (gynecologists, urologists, proctologists, sexologists) and can be used with equally high efficiency for both adult patients and children. Callibri BeFit PRO system is ideal for equipping multidisciplinary centers.

Description

Callibri BeFit PRO is an advanced BFB complex for the prevention and treatment of a wide range of urological, gynecological and proctological diseases, restoration of pelvic floor muscle (PFM) functions, prevention and correction of sexological disorders. The system allows for the preliminary assessment of muscle condition and provides the ability to conduct biofeedback (BFB) training with tracking of the dynamics of recovery efficiency. The BFB method develops the skill of control and regulation of target muscle groups of the patient's body and forms a central program for controlling the physiological functions of the body in the brain

List of available training techniques:

  • electromyography (EMG) using intracavitary electrodes (rectal or vaginal) to record the tone of the PFM;
  • electromyography (EMG) using cutaneous electrodes to record the tone of the PFM in children;
  • electromyography (EMG) using Kolibri skin sensors, designed to monitor the contraction force of non-target muscle groups (gluteal muscles, abdominal muscles, inner thigh muscles) and to develop the correct skill of contracting the target muscle group for a patient;
  • pelvic floor muscle training based on biofeedback (BFB) via electromyogram (EMG).

Kolibri is a universal wireless sensor to which an intracavitary electrode (vaginal or rectal) is connected. The sensor ensures reliable and high-quality recording of the electromyogram and signal transmission (with subsequent interpretation in the game scenario) to the program installed on the computer or laptop.

System and software capabilities

Main features and advantages of Callibri BeFit PRO:

  • the miniature size of the system ensures flexibility and comfort in the work of the specialist with the patient;
  • the wireless design of the complex minimizes artifacts and ensures reliable and high-quality recording of muscle contractions;
  • the received myographic signal is transmitted to multifunctional and convenient software installed on a computer or laptop;
  • control of the tone of the antagonist muscles (anterior abdominal wall, buttocks and inner thigh) during BFB therapy allows us to exclude their reflex involvement and to develop the correct technique for training the pelvic floor muscles for the patient;
  • the data obtained allow the specialist to objectively assess the patient’s current capabilities, select a treatment protocol based on individual characteristics, adjust the parameters of BFB training, dose and control the load both during each session and throughout the entire course of the treatment;
  • special sensors allow the patient to track the state and changes of his/her own physiological processes, which are difficult to feel and control under normal conditions. By observing the signals of the body, the patients learns self-regulation – this is the main task of the BFB method;
  • for maximum efficiency of rehabilitation, game motivation technology is used. For the patient, the program transforms myographic signals into a game. By contracting and relaxing the target group of the perineum, the patient controls the game. In this way, he/she influences the correctness of the training, learns to feel the pelvic floor muscles and, specifically, control them;
  • after each training, a report is generated that clearly and objectively discloses the effectiveness of the work done;
  • as a result of training, the volume, strength, endurance of the pelvic floor muscles, their ability to relax and stretch increase, a habit of controlling the level of muscle tension is formed, and a lasting result is developed.

Software:

  • contains a library of pre-installed courses and trainings developed by practicing specialists and intended for various rehabilitation purposes (health training, training to maintain the tone of the pelvic floor muscles);
  • enables to create a database with an unlimited number of patients with wide navigation capabilities (quick search), filtering and sorting;
  • enables to configure monitoring channels that are not involved in the game process (monitoring channels are used to control the tone of antagonist muscles);
  • enables to objectively evaluate the parameters of the EMG signal on 4 channels. Evaluation of the state of target muscles before and after BFB training enables to objectively confirm the effectiveness of training. The program allows you to register and view the recorded signal, visually compare the results of sessions with each other, and calculate key parameters of the EMG signal: the maximum and average signal amplitude, area, etc.;
  • adapts to a specific user, taking into account individual characteristics (the presence in the program of an expert adaptive system for determining the patient’s capabilities before the start of training);
  • contains a variety of game situations (more than 15 games) that engage the user and make the rehabilitation process more visual and engaging;
  • allows to pause the gameplay (if necessary) during the training (pause mode in games);
  • makes it possible to create user’s own unique methods (training courses) and then save them in the user library;
  • provides the ability to manually (directly during training) and automatically (contains a built-in editor for channel-by-channel creation of arbitrary laws) control the boundaries of the area of successful attempts;
  • enables to work in dual monitor mode: the software functionality is automatically distributed between the specialist’s monitor and the patient’s monitor, which allows the patient not to be distracted from the game situation by physiological indicators that are necessary exclusively for professional analysis;
  • enables to set the time for automatic completion of training (end of training timer);
  • during the training, displays the initial recorded EMG signal and the trend of attempts on the specialist’s screen;
  • generates an extended report on each training session conducted;
  • visually displays the dynamics of indicators within the training course, which allows to evaluate the overall effectiveness of rehabilitation;
  • contains a built-in database of standard templates and phrases for filling in the fields of the reporting form, with the ability to add, save, edit and delete user’s own templates and phrases;
  • enables to monitor the condition and training of selected muscles.
System application areas and primary indications

Urology: various forms of urinary incontinence in women, men and children, neurogenic urination disorders, prostatitis at the initial stage of the disease, chronic pelvic pain syndrome, cystalgia.

Proctology: treatment of hemorrhoids, restoration of sphincter functions after surgery, encopresis (fecal incontinence).

Gynecology: birth injuries, prolapse of the vaginal and uterine walls, overcoming menopause, menstrual pain, prolapse of pelvic organs, preparation for childbirth, postpartum recovery, non-infectious forms of vaginismus.

Sexopathology: in men – increasing the duration of erection, voluntary control and the moment of ejaculation; in women – narrowing the entrance to the vagina (sexual rehabilitation), treatment of anorgasmia, vaginismus, overcoming menopause.

Pediatric urology: all forms of functional urinary incontinence, stress incontinence, nocturnal enuresis.

Pediatric proctology: restoration of sphincter functions after surgeries (in case of damage to the sphincter apparatus), organic fecal incontinence, functional fecal incontinence, anal neuralgia.

Pediatric gynecology: formation and targeted restoration of the pelvic floor muscles after surgical treatment of anomalies and congenital malformations of the genital organs.

Recommendations and contraindications

Processing

Intracavitary electrodes are intended for individual use! After each use, the electrode must be cleaned and disinfected. After use, place the electrode in an individual sterile package until the next session. Chemical treatment of the electrode is allowed. Immerse the electrode with a part of the cable in a 6% hydrogen peroxide solution for 6 hours at room temperature, or for 3 hours at 50 °C. Then wash with sterile water.

Another mode of chemical sanitization is permitted in accordance with MU 287-113.

ATTENTION! It is prohibited to treat the intracavitary electrode with solutions containing active chlorine. To avoid failure of the electrode, do not immerse the contact connectors and plugs in the solution.

Contraindications to the use of BFB therapy:

  • malignant neoplasms requiring treatment (a history of oncology, including neoplasms of the pelvic organs, is not a contraindication for BFB therapy);
  • acute infectious and inflammatory processes – cystitis, colpitis, proctitis, urethritis;
  • myopathy;
  • the first 1-1.5 months after childbirth (at the time of lochia discharge);
  • postsurgical period (on the pelvic organs) the first 4-6 weeks;
  • genital prolapse above grade II, according to the Prof classification POP-Q;
  • chronic diseases at the decompensation stage;
  • febrile conditions;
  • mental disorders at the decompensation stage.
Who can perform BFB therapy with Callibri BeFit PRO?

The BFB training method is intended for rehabilitation procedures to restore the functional capabilities of the pelvic floor muscles and is not diagnostic. Therefore, both a physician (urologist, gynecologist, proctologist, sexologist) and mid-level medical personnel (nurse) can work with the software and hardware complex Callibri BeFit PRO, acting in accordance with the instructions and recommendations of the attending specialist doctor. It is enough to train the nurse in the basic functionality of the software complex, the basic features of the device itself and the principles of the BFB method for the correct and effective operation of the entire urogynecology department.
